I am an in-network provider for Premera. Individual benefit plans differ in the amount the patient is required to pay per visit, the number of visits covered, and the form of billing required. If I am in-network with your insurance provider, I will submit your claims directly, however some services require prior authorization. If I am not in-network with your insurance provider, I can provide you with the necessary billing documents to submit your claims for reimbursement.
Please note academic testing is generally not covered by medical insurance benefits.
